For submission of service provided as part of warranty, please fill out fields below denoted by asterisk (*) and attach clear photos/scans as JPG, PNG, PDF in a size not greater than 1 Mb of the following documents:

  • completed complaint report
  • proof of purchased boiler (VAT invoice or receipt)
  • completed warranty card
  • boiler commissioning report
  • proof of boiler maintenance with proof of payment (VAT invoice or receipt)
  • boiler nameplate

If you complain against improper combustion (such as no draught, tar forming, emission of smoke into boiler room), please attach a clear photo/copy of chimney expert opinion confirming that a flue duct fulfils all the requirements for specific boiler sizes included in the maintenance and technical documentation.
